Yes! It was in the original "The Twilight Zone" in B&W. I think I have it on DVD (but maybe not). It's positively frightening, and it looks just like you!(*)

There was a less scary one, more like a morality play, which included the ability to rejuvenate the body as well. I don't quite like that one.

That's the tagline the ep is sometimes known for ("look just like you!") I think at one point the antagonist refers to Shakespeare as "smut."
