Finally spoke to urologist friend, and second urologist (neighbor). Friend considers the possibility that over the years President’s physicians deferred doing rectal exams thus delaying finding the prostate cancer early. As far as the PSA test, he disagrees with the recommendation that men over 70 should not get PSA tests. Neighbor concurs. All 3 of us believe that having the knowledge that you have the cancer, even into your 70’s, is better than not knowing. Biden may be a good example. I now know 3 nonagenarian men, all active and with good qualities of life. Had they had an ignored prostate cancer diagnosis at 72 they may not be here.
Both urologists also believe it is certainly possible he has known about his cancer for years, but did not release the information, and chose not to have it treated. That would not be that unusual, as it is usually slow enough in its development to often never metastasize during your lifespan. And had he been treated, that’s a dozen more possible loose lips.

Aren’t those recommendations for the general public an average based on some probability? In general many cancers would be caught earlier if you ignored the recommendations and went earlier or more often than necessary.

Yes. I’ll specifically address prostate cancer. My high PSA led to an MRI. MRI showed small lesions. To get an accurate diagnosis you need a biopsy. The biopsy needle is in that messy area down there…. and there is a 1% chance of a serious, even life threatening sometimes, infection. So if 100,000 more men get these biopsies, that’s 1,000 more serious negative outcomes. For the ones that proceed to surgery, there’s a whole new bunch of potential complications. If you choose radiation it’ has its own set of negatives.

Colon cancer, as an example, if found early, has an extremely high cure rate. So I’ve just scheduled my (hopefully useless!) colonoscopy.

Too long to go into fully, but if you test everyone for everything everyday you will get a lot of ‘false positives’ which if treated lead to their own set of problems.

It is certainly possible that Biden knew beforehand and didn't tell anyone. But, it's more likely nobody looked for it because the recommendation was not to. To assume he knew and didn't tell anyone, well let's just say extraordinary claims require extraordinary evidence. And right now there is no evidence he knew beforehand.
